Most mid-market healthcare organizations eventually hit a wall where the IT Director can no longer perform the security function alone. When security needs executive-level attention, there are three ways to get it: a virtual CISO (vCISO), a fractional CISO, or a full-time hire.
A full-time CISO is the traditional path, but at $300,000 – $400,000+ per year fully loaded, it’s an expensive one. The terms vCISO and fractional CISO often get used interchangeably, but they aren’t the same thing. A vCISO is a service: a defined set of deliverables that may include a risk assessment, risk register, policy support, and executive reporting, delivered by a provider’s team on a set cadence. A fractional CISO is a person: a named, experienced security executive who takes a seat on your leadership team part-time and acts within the authority you delegate. Both cost a fraction of a full-time hire.
Here’s how the three options compare, how to tell which one fits, and how to vet a provider.
vCISO vs. Fractional CISO vs. Full-Time CISO
A full-time CISO is a permanent executive embedded in your daily operations. They attend leadership meetings, manage your security team directly, respond to incidents in real time, and own security as their only job.
A fractional CISO fills that seat part-time. They join leadership meetings; shape the security strategy, roadmap, and budget; and present risk to your board, working an agreed-upon number of hours each month. Final decisions stay with your executives.
A vCISO doesn’t take a seat on your leadership team. Instead, it gives your designated security official a structured program and an expert team behind them. Because a provider’s team works across multiple clients, they also bring a broader view of emerging threats and what’s working elsewhere.
The real question is how much leadership you need.
| Factor | vCISO | Fractional CISO | Full-time CISO |
|---|---|---|---|
| What you get | A provider’s team delivering a defined set of security deliverables on a set cadence | A named security executive on your leadership team, part-time | A permanent, dedicated security executive |
| Annual cost | $60K–$120K for most mid-market healthcare retainers ($3K–$15K per month) | Scales with the hours allocated each month | $200K–$350K base salary; $300K–$400K+ fully loaded |
| Recruiting cost | None | None | 20–30% of first-year compensation |
| Tenure risk | Team-delivered, so it doesn’t depend on one person | Provider replaces the individual if needed | Average tenure of 18–26 months |
| Availability | Scheduled meetings and reporting during business hours | Agreed hours each month, part-time | Full-time, on-site |
| Leadership role | Advises and supports your designated security official | Participates in leadership and board meetings; acts within the authority you delegate | Member of your executive team |
| Breadth of experience | Healthcare expertise across multiple clients | Healthcare expertise across multiple clients | Deep knowledge of one organization |
| Compliance | Risk assessment, risk register, and reporting your compliance program relies on | Leads your security and risk program; final decisions stay with your executives | Full program leadership |
| Board reporting | Monthly executive reporting; board presentations available | Participates in board and committee meetings | Ongoing and embedded |
The tenure figure matters more than most organizations expect. With CISOs averaging less than two years in a role, many companies end up paying recruiting and onboarding costs every two to three years. When you work with a vCISO or fractional CISO through a provider, continuity is built into the engagement instead of depending on one person.
How to Know Which Option Fits
A vCISO is the right fit if:
- You have a designated security official, often your IT Director, who needs a structured program and expert support behind them.
- You need a current risk assessment and regular security reporting for leadership, auditors, or insurers.
- You have specific security objectives that need addressing from someone with time-tested expertise.
- You’re building a security program from scratch.
A fractional CISO is the right fit if:
- Your organization needs executive-level security leadership, but the budget can’t support a full-time CISO.
- You want a security leader in the room for leadership meetings, budget decisions, and board conversations.
- You’re going through a merger or acquisition and need to combine different security programs.
- You need interim leadership after a CISO departure or during recovery from a breach.
Ask these questions internally. If you’re a “no” to two or more, it’s time to bring in outside security leadership, through either a vCISO or a fractional CISO.
- Is your IT Director able — in both capacity and capability — to handle security on top of other responsibilities?
- Could you show a board member, auditor, or cyber insurer a current risk assessment and tested incident response plan today?
- Do you know where patient data lives across your systems, devices, and third-party vendors?
- If ransomware disrupted clinical systems tomorrow, would everyone know who leads the response and what happens first?
- Does leadership get regular updates on security risk?
What to Look for in a vCISO or Fractional CISO
With a vCISO, you’re evaluating a provider’s team and program. With a fractional CISO, you’re evaluating that team and the specific person who’ll sit at your table. Either way, look for:
Hands-on incident response. A leader who has worked through real breaches brings calm, structured direction to a crisis and knows how to coordinate legal, PR, and forensic investigators. Look for someone who has led containment and recovery.
Healthcare-specific expertise. Security priorities vary widely by sector. Healthcare security is about protecting patient data and keeping care running. That means safeguarding electronic health records, medical devices, and the systems clinicians depend on. A CISO who already knows your threat landscape and frameworks needs almost no ramp-up time. They should also understand risks specific to healthcare, such as ransomware that disrupts patient care, vulnerable connected medical devices, third-party vendor exposure, and breach notification requirements.
Boardroom literacy. A great CISO is both a business leader and a technician. Look for a track record of securing budget, presenting risk to boards in financial and operational terms, and aligning security strategy with business growth.
The right mix of strategy and hands-on skill for your objectives. If you already have core security tools and systems in place, look for someone strong in compliance, policy, and risk management. If you’re still building the basics, look for someone who can help design your systems and choose the right tools.
Questions to Ask Before You Choose a Provider
To learn how deep a provider’s experience really goes, ask for specific scenarios:
- “Is this a defined program delivered by your team, or a named person on our leadership team? What decisions would they be authorized to make?”
- “Walk me through a time you had to tell a CEO that a product launch needed to be delayed because of a security flaw.”
- “What is your exact playbook for the first 30 days in an organization with low security maturity?”
- “Describe a breach you managed. What went wrong, and how did it change your approach to security?”
- “If you become unavailable, what happens to our engagement, and how quickly can a replacement get up to speed?”
The strongest answers will be specific and honest.
